My stats are similar to yours (AMG, higher step 1, lower step 2, also bottom of the class). I have invites from a few small community programs and 1 low(mid?) tier academic program all in the south. I'm from the southeast(but have strong ties to Cali) and I applied to most of the community programs in Cali. I haven't heard anything back from any of the Cali schools. Those programs are notoriously competitive, I'm not sure how I will fare, thus I applied to 35 additional programs.
One thing I think may help is priming your letter writers/advisors who have connections to programs by letting them know you may need them to make calls and emails on your behalf. That way if you if you have to ask them beg for interviews a month down the road it won't be awkward. Also, let them know where you stand in the class so they know why you may have trouble getting interviews. Most of my letter writers were surprised about my class rank since they thought I was a strong clinical student and said they would be more than happy to make a call or email to people they know at other programs. I have atleast 6 different programs my advisors will be able to put in a word for me at if I don't initially get an interview. I'm waiting a month until I use this option.
Also, at this stage the top students are getting interviews at top programs across the country. These are students that could have matched any specialty, but chose IM for some strange reason. Ignore these people and life will be less stressful.
