What do I need to do to get in?

I'm currently a working cardiovascular perfusionist (run the heart/lung machine during heart surgery) that is currently looking to career change into dentistry. I am looking into career shifting due to not enough patient interaction (patients are asleep while I'm working), no personal satisfaction with my work, cardiovascular surgery is rapidly evolving in a way that will lower the amount of work for my profession, and I need to be on the top of the totem pole in my job. I have an uncle who is a practicing dentist and he recommended dentistry when I expressed my frustrations with my current job. He also hinted that he's looking for a family member to give his practice to in 10 years. He had my attention so I shadowed him for a week straight and loved it! Sadly I have a fairly big downfall... my undergrad gpa. I did not try in college, honestly, and finished with a 3.0gpa (don't remember science but probably similar) with a biology major and chem minor. I can confidently say that my gpa does not reflect my academic potential. I have always been able to get an A on any test whenever I felt like I needed to give my grade a little bump. Luckily I was able to pair my gpa with a GRE score in the 95th percentile to get me into my masters program for cardiac perfusion. I finished my masters with a 3.5 gpa (also not trying.. just simply getting by). I have finally found something that I am willing to give my all to get into but I'm not sure how to proceed. I'm planning on studying for the DAT and taking it mid-summer with the goal of scoring at least a 23. This will be my checkpoint, I won't take classes to bring up my gpa until I have a DAT score that stands out. Several of my prereqs for dental school are over the 5 year time span so I will need to retake them. What do you guys think is the best way for me to proceed with this? Should I retake all of the prereqs, get A's, and then apply or since my undergrad gpa is so low, do a post-bacc program and then apply? I'm planning on calling the admissions office for my state school tomorrow to get their thoughts but would love to hear from you guys! Thanks in advance for any input/advice. Please be completely honest, no sugar coating this. I need to know what it will take to get me in.
